Hello, I am new to this forum. I would also like to thank dr rassman for having a blog and forum

First of all I am 23 years old with thinning hair. I went to see my dermatologist and he prescribed me propecia.
I had a couple of question, but he was in such hurry that day I coudnt ask him. That is why I am reluctant in using propecia.
Here are my question
If DHT,the by-product of testosterone causes hairloss, then why does propecia(A known DHT inhibitor) aggravate more hairloss in some people and aggravate more hairline hairloss? Is it because the increase in testosterone cause shedding or just the natural testosterone by itself? If thats case then why take propecia. If propecia will only inhibit DHT and not testosterone then the testosterone will cause hairloss. That is why I am scared to start using propecia, becuase I am scared that it might make my hair even worse. My hair is not noticibly thin, but I am losing hair, and Im just scared that if I take propecia I might aggravate more loss and make my hairloss even worse.
I also suffer from acne and I take topical solutions to correct that. My other question is I have heard that propecia sometimes as a rare side affect causes acne in some individuals.
I have heard that acne is associated with DHT as well, then shouldn't propecia help clear acne then cause more acne? If DHT is being inhibited by propecia then shoudnt that help the acne? Instead I see people complaining that it cause more acne and pimples.
These are the reason why im scared to take propecia and I am really stressed out right now over this. Please someone help me clarify this.

I think your fears are considerably unfounded.
Testosterone does not cause hair loss, DHT which is created by Testosterone is what causes MPB in some men who are genetically predisposed to it.
Some people taking Finasteride develop what's called "Shock loss" when they start the drug. This is rarely permenant and the hair usually grows back in time, and grows back thicker.
I've never heard of anyone taking Finasteride and having their hair loss come out worse. If anything, it simply just continues as it would have, or slows down completely and in some rare cases, actually reverses.

The answer you seek is right in your own question.

The side effects of Finasteride are fairly benign, and they go away once you stop taking the drug.
Most people we see in the office who take Finasteride have no side effects at all. If you see side effects that you don't think are acceptable, simply stop taking the drug. Problem solved.
It seems right now you're kinda driving yourself a little crazy trying to go through every single possible circumstance that may come up if you start taking this drug, despite the fact that any of these side effects almost never occur all together in one person, nor do they even usually occur singularly.
The only way you're going to know whether any of these side effects are going to occur in you is if you start taking the drug. You can ask as many questions to as many different people on the internet, but nobody will ever be able to guarantee exactly how this drug will effect you. But looking at the scientific data on it, I think you're worrying too much.

Thank you mark i really really aprreciate the the fact that you replied. I really do want to start taking propecia except I was just scared.
I know your right about the fact that a perosn will only know the answers simply by trying propecia.
One last thing.
My hairline is receding and is thinning around the temples but it is not noticible yet can propecia cause some more damage if the drug hasnt been known to help hairlines and temples. I know some of my question are ridiculous but I have searched numerous blogs and forum and even blogs from balding blog where people compalin that propecia killed there hairline or made it worse than it was before taking propecia

There are no known scientific studies that show that Propecia can cause more hair loss by taking it. At the very extreme worst, all we've seen is that a very small percenatge of some people may get some shock loss which returns fairly quickly.
Because Propecia isn't shown to reduce hair loss at the hairline as well as it does for the crown is no indicator that it will make you lose hair at the temples or hairline.

You're right, that is contrary to popular belief, as well as all the scientific knowledge we have about hair growth.

I and others are regrowing our scalp hair by removing our body and face hair from the follicles. This gets the needed nutrients to grow hair flowing to the scalp again, instead of being wasted on unwanted hair. The nutrients include dht and 5 a-reductase.
It is not possible for to much of anything to cause pattern baldness, the fact that AGA occurs only in some people, always after puberty and always in the same pattern tells us that. If it is true that dht affects only scalp hair follicles, those who lose hair only at the crown, have only scalps at the crown, those who lose at the temples have scalps only at the temples, those whit diffuse balding have intermitent scalps, those who bald all over the top of the head have large scalps, and those who do not bald have no scalps. I have been using propecia since I was thirty. My maternal grandfather, as well as my older brother, and all my maternal uncles are completely bald, many since their thitties or even twenties. My hairloss completely stopped within weeks of starting, and has slowly reversed over the last 10 years, even slightly recovering at the forehead hairline. I have used minoxidil with retin a at the hairline once for a few months and saw fairly dense and rapid regrowth at the hairline. Propecia stops hairloss for me. Everything that has been clinically demonstrated has turned out true for me. I am now trying nizoral, and will re-try minoxidil+retin A to regrow my hairline when I find a good source of it without propelyne glycol at a good price.

Yes it is true to an extent. Propecia can aggravate more hair thinning at the hairline due to the testosterone rise. The hairline is more sensitive to testosterone than DHT. Its a gamble. You save your crown area at the expense of your hairline. Very risky
